This resource defines the patient involved in the remote clinical monitoring. Details like patient's name contacts gender birth date country code and preferred languages will be used. The institution (i.e AMK Polyclinic) that manages the remote clinical monitoring for the patient and the group organization (i.e National Healthcare Group Polyclinics) that owns the patient record will also be stored.

This bundle is used to hold 2 entries Patient and CarePlan resources for the creation of the patient and his her assigned clinical programme in a single transaction

This resource defines the vital signs readings of the patient. Details like the type of vital signs (i.e blood pressure) the readings for each vital sign (i.e systolic blood pressure diastolic blood pressure) the device guid that took the readings and the subject patient id. In the event the readings are missed the effective period will be used to store the start and end date of the missed readings with the data absent reason as unknown .
